The word microdosing gets used in a lot of contexts, most of them having nothing to do with testosterone. But within TRT, it has a specific and increasingly well-supported meaning: administering smaller amounts of the hormone more frequently, in order to maintain stable serum levels rather than creating the peaks and troughs that weekly injections produce.
It is not a fringe concept. It is not a shortcut. It is, in fact, closer to how the body naturally produces testosterone than the delivery formats most men are currently using.
What Microdosing Actually Means in a Clinical Context
The testes do not produce testosterone once a week in a large bolus. They produce it continuously throughout the day — roughly 3 to 10 mg daily in healthy adult men — in response to pulsatile LH signaling from the pituitary. The result is a serum testosterone level that stays relatively stable across the day, with mild circadian variation (higher in the morning, slightly lower by evening) but no dramatic weekly swings.
Microdosing in TRT attempts to replicate this pattern by distributing the total weekly testosterone dose across more frequent, smaller administrations. The goal is a flat pharmacokinetic profile rather than a sawtooth wave.
The Two Main Approaches
There are two practical ways to achieve more frequent testosterone dosing: more frequent injections, or daily transdermal delivery.
Injection microdosing means moving from weekly to daily or every-other-day subcutaneous injections, typically using small insulin syringes. A patient who was injecting 100 mg once weekly might instead inject 14 mg daily. The total weekly dose is identical. The delivery profile changes substantially: smaller injections into subcutaneous fat absorb more slowly than intramuscular injections, and daily administration eliminates the weekly peak-trough cycle entirely.
Daily transdermal delivery achieves the same pharmacokinetic outcome through a different mechanism. Instead of injecting testosterone, the hormone is absorbed through the skin continuously. Applied once daily, modern transdermal formulations produce serum testosterone that rises modestly after application and remains relatively stable across the 24-hour period. The peak-to-trough variation with daily transdermal is approximately 2.7 pg/mL for free testosterone — compared to 26.7 pg/mL with weekly intramuscular injections.
Why This Is Gaining Ground
The interest in testosterone microdosing reflects three converging realities. First, the side effect profile of weekly injections — erythrocytosis, estrogen spikes, mood instability tied to the injection cycle — is increasingly well-documented. Second, the cardiovascular safety data showing transdermal delivery outperforms injections has become harder to dismiss. Third, patients are increasingly aware that the cycle they experience — feeling good on injection day, worse by day six — is not an inevitable feature of TRT. It is a feature of their delivery method.
What the Clinical Evidence Shows
Daily transdermal testosterone has the longest and most robust clinical record of any frequent-dosing TRT format. It is FDA-approved, Endocrine Society-endorsed, and was the delivery method used in the TRAVERSE trial, which established the cardiovascular safety of testosterone therapy with no increase in major cardiovascular events.
The data on injection microdosing is more recent and less voluminous, but consistent with the pharmacokinetic prediction: more frequent subcutaneous injections produce flatter serum profiles, lower peak-to-trough ratios, and reduced erythrocytosis compared to weekly intramuscular protocols. The drug is identical. The outcomes differ because the delivery pattern differs.
The Semantic Overlap with Consumer Wellness
It is worth acknowledging that the term microdosing has acquired broader cultural associations in recent years, particularly in wellness contexts. In TRT, the meaning is purely pharmacokinetic: distribute the dose more frequently to achieve stable steady-state levels. There is no mysticism here. It is simply applied pharmacology — matching delivery frequency to physiological rhythm.
The concept is gaining ground not because it is trendy, but because the clinical rationale is straightforward and the evidence behind it continues to accumulate.
