Dosing Women Differently

Women require 10–15 times less testosterone than men — a dosing range where precision matters enormously and supraphysiologic exposure causes virilization.

There is a small but important pharmacological fact that explains why you cannot simply hand a woman a man’s testosterone prescription and tell her to use less of it. The fact is this: the therapeutic window for testosterone in women sits between roughly 15 and 50 ng/dL in total serum testosterone. The therapeutic window for men sits between 400 and 700 ng/dL or higher. Those ranges differ by a factor of roughly 10 to 15.

This is not a minor calibration adjustment. It is a different problem entirely.

Why Doses Are So Much Lower

Testosterone is biologically active at concentrations proportional to the receptor density and sensitivity of the tissues in question. Women have androgen receptors throughout their bodies — in brain, muscle, bone, skin, and reproductive tissue — but those receptors are operating at a different baseline set point. A woman’s physiology is calibrated for a testosterone environment in the 15–70 ng/dL range. A man’s physiology is calibrated for 300–1,000 ng/dL.

Giving a woman a male-scale dose of testosterone does not double the therapeutic effect. It overshoots the target by an order of magnitude and causes problems.

The standard male testosterone gel dose might deliver 50 mg of testosterone per day, targeting serum levels of 400–600 ng/dL. The same gel at the same dose applied to a woman would produce dramatically supraphysiologic levels — potentially 10 to 20 times her normal range — and that is where virilization becomes a real concern.

What Virilization Actually Means

Virilization is the clinical term for the development of male secondary sexual characteristics in a female body as a consequence of androgen excess. At supraphysiologic testosterone exposure, the following can occur:

  • Hirsutism: excess body and facial hair, driven by androgen sensitivity in hair follicles, particularly on the upper lip, chin, sideburn area, chest, and abdomen.
  • Acne: sebaceous glands are androgen-responsive; testosterone stimulates sebum production, and excess testosterone dramatically increases acne risk.
  • Voice deepening: a more permanent effect involving hypertrophy of the laryngeal cartilage. Unlike hirsutism, this does not fully reverse when testosterone is discontinued.
  • Clitoral enlargement: also partially reversible with dose reduction, though some degree of change may persist.
  • Male-pattern hair loss: paradoxically, while androgens stimulate hair follicle growth on the body, they have the opposite effect on scalp follicles that are genetically susceptible to androgenetic alopecia.

The important distinction is that these effects are dose-dependent. They are not inherent risks of testosterone use in women at physiologic doses — they are consequences of excessive dosing. At doses targeting premenopausal physiologic ranges (15–50 ng/dL), the frequency of androgenic adverse effects in clinical trials has been low. The Huang et al. (2014) TDSM trial specifically monitored for these effects using structured instruments — the Ferriman-Gallwey scale for hirsutism, Palatsi scale for acne, caliper measurements for clitoral size, sebum tape measurements for skin oiliness — and reported that “frequency of androgenic adverse events was low,” even in the highest dose groups.

The goal of female testosterone therapy is not to suppress the androgen environment — that would defeat the purpose — but to restore it to a range that is normal for women, without exceeding it.

Practical Dosing: The Numbers

In clinical practice, female testosterone therapy typically delivers testosterone in concentrations and doses far below what male patients receive:

  • Transdermal gels (male formulation, off-label): 1–5 mg/day, compared to 50 mg/day for male patients. This requires either significant dilution or very small application volumes, neither of which is ideal from a dosing precision standpoint.
  • Compounded transdermal creams or gels: typically prepared at 1–2 mg/mL or 0.5–1% concentration, with prescribed doses of 1–5 mg/day applied to thin-skin areas (inner wrist, inner forearm, labia majora depending on preparation).
  • Testosterone patches (where available): the Intrinsa patch delivered 300 mcg (0.3 mg) per day, calibrated specifically for female physiology. This is the kind of dose precision that disappears when you are working off-label with male-formulated products.
  • Pellets: used by some practitioners, but present challenges in reversibility and dose adjustment.

The target for most postmenopausal women on testosterone therapy is a total serum testosterone in the premenopausal physiologic range: roughly 15–50 ng/dL when measured by LC-MS/MS. The goal is not to maximize the number — it is to restore a normal range, confirm symptom improvement, and monitor for excess.

The Role of Compounding Pharmacies

Because no FDA-approved testosterone product for women exists in the United States, compounding pharmacies play a central role in this therapeutic space. Compounding pharmacies can prepare testosterone in concentrations appropriate for female dosing — 0.5 mg, 1 mg, 2 mg, or other custom strengths — in base formulations designed for efficient transdermal absorption.

This approach has genuine advantages: dose customization, flexibility in delivery vehicle, ability to combine testosterone with other hormones in a single preparation if clinically appropriate. It also has genuine limitations. Compounded preparations are not subject to the same FDA manufacturing standards as approved drug products. Quality control varies across pharmacies. Absorption kinetics differ between compounding bases, meaning the same nominal dose from two different pharmacies may not produce the same serum concentrations.

This is precisely why blood monitoring is not optional in female testosterone therapy — it is a clinical necessity.

Blood Monitoring in Women on T Therapy

Monitoring protocols for women on testosterone therapy typically include:

Before initiation: baseline total and free testosterone (by LC-MS/MS if possible), SHBG (sex hormone-binding globulin), complete blood count, liver function tests, and lipid panel. Baseline assessment of symptoms using a validated instrument (such as the Profile of Female Sexual Function or the Female Sexual Function Index) helps establish a baseline for comparison and tracks treatment response.

At 6–8 weeks after initiation: serum testosterone to confirm that the dose is achieving the target range without exceeding it. If levels are too low, the dose or delivery method may need adjustment. If levels are approaching or exceeding 50 ng/dL on a consistent basis, dose reduction is warranted.

Ongoing (every 6–12 months): testosterone levels, hematocrit (testosterone can stimulate red blood cell production, though this effect is much less pronounced at female doses than male doses), lipids, and symptom reassessment. Monitoring for signs of virilization at each visit.

The serum testosterone assay method matters. As discussed in previous posts, standard immunoassay-based tests have poor accuracy at female concentrations. LC-MS/MS is the gold standard. Clinicians using standard lab testosterone panels for female testosterone monitoring are working with a less reliable instrument than they may realize.

The Design Logic of Female-Calibrated Products

This is where the pharmacology connects directly to the clinical product design in testosterone therapy for women. The products that exist specifically for female dosing — whether compounded or the few international options — are built around one central constraint: delivering enough testosterone to restore physiologic activity in androgen-responsive tissues, without the concentrations needed to trigger virilization or the dose levels associated with cardiovascular risk signals.

Topical delivery systems calibrated for female dosing — 0.5 mg, 1 mg, 2 mg per application — are not simply downsized versions of male products. They represent a different pharmaceutical problem. The delivery system must be efficient enough that a very small absolute dose produces consistent, measurable serum concentrations in the 15–50 ng/dL target range. Too little efficiency and the dose does nothing. Too much and you overshoot.

Getting that calibration right is the pharmacological challenge behind every dose strength designed for women. It is also why the absence of an FDA-approved female testosterone product is not merely a regulatory paperwork problem — it is a clinical gap that has material consequences for how accurately women can be treated.
